Act as the main point of contact for residents' families.
Handle enquiries, feedback, and complaints professionally.
Organize regular family meetings, visits, and communication updates.
2. Community Outreach & Networking
Build relationships with hospitals, clinics, NGOs, religious bodies, and local communities.
Coordinate referral processes and support new resident admissions.
Promote the nursing home's services through outreach programs, events, and partnerships.
3. Events & Activities Coordination
Plan, organize, and oversee social, recreational, and festive activities for residents.
Work with caregivers, nurses, and volunteers to ensure activities meet residents' physical, emotional, and social needs.
Encourage participation from community volunteers, schools, and external organizations.
4. Communication & Administration
Prepare reports on community activities, engagement levels, and resident participation.
Manage schedules for community programs and maintain necessary documentation.
5. Resident Well-being Support
Help ensure residents feel socially included and emotionally supported.
Identify residents at risk of social isolation and arrange appropriate interventions (e.g., counselling, family contact, or group activities).
Collaborate with nurses and caregivers to monitor residents' social and emotional progress.
6. Liaison Management
Provide regular updates to management on community activities, challenges, and opportunities.
Suggest improvements to enhance resident satisfaction and strengthen family relationships.
Qualifications & Requirements
Diploma or Degree in Social Work, Psychology, Communications, Community Development, or related field.
Experience in community engagement, event coordination, or social care is an advantage.
Strong interpersonal and communication skills.
Empathetic, patient, and able to work with elderly individuals.
Good networking skills and confidence when dealing with external partners.
Proficient in Microsoft Office and basic reporting.
