1. Receive report and clarify concerns at the beginning of shift.
2. Learn and practice safe patient handling to promote wellbeing of patient and self.
3. Document with responsibility and accuracy - patient condition, information on the work area, reports and charges of articles and items used in patient care.
4. Implement care, and maintain patient records as per hospital policy and with objectivity.
5. Demonstrate technical competency in performing procedures that are an extension of the nurses' role.
6. Administer medication and injections following the six principles of medication administration.
7. Carry out all patient care procedures such as per hospital policy to ensure the best outcomes.
8. Assess patient for skin integrity, pain and fall sick. Trigger required consultation and ensure documentation of any changes from the norm.
9. Perform all required re-assessments every shift including and document clearly, report variance to Charge Nurse/Supervisor.
10. Provide patient education after establishing discharge planning in collaboration with patient and family from the day of admission.
11. Integrate knowledge into practice with critical thinking and take responsibility for decisions made in the care of patients.
12. Provide orientation to patient regarding room facilities, meals, and visitor regulations as well as information that may be asked for.
13. Perform comfort rounds and ensure ongoing communication with patient on all that pertains to his/her care.
14. Maintain associated patient records in a clear legible and organized manner.
15. Document all relevant information clearly and provide accurate verbal reports when patients are moved from one area to another.
16. Take responsibility to learn about new medications, physician preferences and initiate credentialing process
17. Interact amicably with personnel from other departments to facilitate quality patient care.
18. Coordinate tasks and abilities using procedure guidelines and policies of GTSH.
19. Initiates emergency resuscitative measures according to adult resuscitation and advanced life support protocols.
20. Maintains confidentiality in matters related to patient, family and client facility staff.
