Job Description
Position Purpose To provide educational information to patients to support them in disease management and understanding of their treatment while working closely with the patient care lead on the day-to-day operations of the program(s).
You will be responsible for the provision of information support and care to patients on treatment programs. Assist patients with educational needs, problem resolution and health management across the program life cycle. Assisting the patient care lead to the enrolment of the patients in the program. Ensuring current patients receive their drugs quickly and efficiently. Liaising with pharmacies to ensure timely delivery of patient medications. Ensuring that eligible patients are enrolled in the program in a timely manner and update on the program life cycle. Developing patient care plans and effectively implementing them. Conducting administrative duties
You are required to hold a Degree in Nursing and have worked in patient management for 2-3years, dealing with patient support on a daily basis. You will need to have good reporting, MS Office/Excel and administration skills. Experience in data monitoring and management is also required. The position is based in Malaysia and it does entail frequent travel to patients, clients, partners and Pharmacies which may be outside your designated region. Flexibility to travel, as well as working hours, is essential
