Job Description





We are looking for suitable and qualified candidate for the following position:

PHARMACIST
(CRITICAL CARE CLINICAL PHARMACIST) PHARMACY & SUPPLIES DEPARTMENT

Responsibilities

  • Provide expert level of consultation, guidance, and direction for appropriate critical
care-affiliated medication use.
  • Optimize the outcomes of critically ill patients by providing evidence-based, patient-
centered drug therapies, and monitor for drug-related problems and medication
cost.
  • Work with the critical care team to provide complete care to the patient through
inpatient ward round services, consultative services, and multidisciplinary patient
care meetings.
  • Work with other pharmacy staff to provide structured, contemporary and
comprehensive training for pharmacy, nursing, and medical staff and students.
  • Lead and participate in interdisciplinary hospital committees, protocol development,
and related accreditation activities.
  • Participate actively in all respective area of specialty team meetings, scientific
research, and educational teaching conferences.
  • On occasions provide pharmaceutical care in Medication Reconciliation areas, INR
Clinic, and Multidisciplinary Clinic/s.
  • On weekends support supply medications to outpatients or inpatients or patient care
areas.
  • Participate in all departmental & organizational-wide quality assurance activities.
  • Participate in educational and training activities for professional development.
  • Perform inventory control & monitoring.
  • Perform other duties assigned by the supervisor.
Requirements

  • Possess Degree in Pharmacy (recognized by Malaysian Pharmacy Board).
  • Registered with Malaysian Pharmacy Board.
  • Post graduate certifications desirable.
  • At least three (3) years\xe2\x80\x99 experience as a Clinical Pharmacist including Critical Care
wards.
  • Experienced in implementation of service/process/quality improvements in hospital
setting and high-quality data collection and analysis.
  • Possess knowledge of pharmacy practices & procedures and pharmacy
generalized inventory system & other procedures related to drug supply.
  • Proficient working knowledge of Health Information System / Pharmacy software
& Microsoft Office.
  • Leadership capabilities to schedule & monitor subordinates\xe2\x80\x99 work.
  • Good communication skills (verbal & written).
