Preferably 3 years of working experience as Pharmacy Assistant in hospital setting
Can speak in English, Bahasa Malaysia and Mandarin
ResponsibilitiesDispensing Related Duty
Patient registration.
Order entry.
Screen & verify prescriptions.
Confirm & clarify any ambiguity, over dosage, underdosage, polypharmacy, contraindication, etc. with prescriber, nurse or pharmacist.
Cross-check medications prepared prior to dispensing.
Dispense medication in accordance with Good Dispensing Practice, under a pharmacist\'s supervision.
Provide patient medication and/or device counselling.
Update take-home drug & liaise with ward to discharge patient, if need arises. Perform bedside dispensing & counselling, if need arises.
Charge prescription manually, during system downtime.
Receive & count return stock, issue credit note (refund note) if relevant. Ensure all documentations are completed.
Issue \'Pending Drug Request\xe2\x80\x9d in stock-out or insufficient stock situation, or \xe2\x80\x9cKIV Dispensing List\' for partial supply of medications to patient.
Ensure that the patient is promptly informed about the availability of \xe2\x80\x9cpending drug\xe2\x80\x9d & arrange for collection.
Ensure thermolabile drugs are dispatched in cold bags with cooler pack.
Inventory Control
Ensure sufficient stocks are available in pharmacy. Inform Store staff, Purchasing staff or Senior Pharmacist (Purchasing) on which stock is physically low in stock.
Check accuracy in physical stock issue & electronic stock transfer. Report discrepancy to Store staff, if any.
Check & adjust stock expiry date, when assigned.
Perform electronic stock adjustment (SA in) / ad-hoc receive (AR) to adjust in stock quantity for urgent charging to patient and reverse the adjusted quantity (SA out) / perform supplier goods return (SR) once the actual stock is available.
For other stock adjustments, to submit the Stock Adjustment Request Form to Senior Finance Manager before the adjustment.
Ensure all stock are properly & alphabetically arranged at appropriate storage areas (e.g. shelves, drawers, locked cabinets, refrigerator etc.), in first-expired-first-out (FEFO) order.
Lend / Borrow stock to / from internal clinics & document accordingly. Update in record book once loan stock is returned to / from the clinic. Remind borrower to return stock.
