About this role

This is a placement for a client of Straat-X ICT Sdn Bhd, who is looking for

Remote Registered Nurse

position. We are a U.S.-based healthcare organization partnering with providers and pharmacies to deliver scalable, device-agnostic

Remote Patient Monitoring (RPM)

and

Chronic Care Management (CCM)

solutions. Our offshore team provides high-quality, cost-effective patient engagement and clinical support under the supervision of U.S.-licensed providers.

Key Responsibilities

Clinical Oversight

Monitor patient dashboards for abnormal vital signs (e.g., hypertensive crises, hypoglycemia). Conduct triage via phone or video calls and escalate to U.S. providers when necessary. Review and validate Medical Assistant notes for documentation accuracy.

Patient Care

Provide patient education on chronic disease management (hypertension, diabetes, COPD, CHF). Conduct monthly "interactive communication" calls required for RPM/CCM compliance. Develop and maintain individualized care plans in collaboration with U.S. clinicians.

Case Management

Assign tasks to Medical Assistants and ensure timely handling of escalations. Track patient adherence to ?16 device readings per month and intervene when patients fall behind. Support medication reconciliation, symptom follow-up, and ongoing chronic care monitoring.

Compliance & Quality

Ensure documentation meets U.S. Medicare/Medicaid billing requirements (CPT 99457/99458, 99490/99439). Participate in quality audits and provide workflow improvement feedback. Safeguard Protected Health Information (PHI) in compliance with HIPAA and company policies.
Skills and Qualifications

Active

Registered Nurse (RN) license

in the Philippines (in good standing) (Compulsory). Minimum

3 years of nursing experience

, including at least 1 year in telehealth, case management, or chronic disease care (Compulsory). Strong Chinese language proficiency, with ability to communicate clinical findings clearly (Compulsory). Familiarity with U.S. healthcare documentation standards preferred (Compulsory). Reliable internet connection and secure remote work setup. Clinical triage and decision-making. Chronic disease education and management. Leadership and task delegation (guiding Medical Assistants). Telehealth communication and patient engagement. Strong documentation and compliance awareness.

Pay: RM3,000.00 - RM5,500.00 per month
