Assist the Manager, Patient Solutions on patient care program and materials.
General Responsibilities
To deliver excellent patient support that follow the DKSH protocols and program materials.
Provide patient and/or caregiver support via telephone and other support channels (inbound/outbound) to address patient questions and concerns about their disease, prescribed medication, psychosocial and family concerns, communications with their healthcare provider, care partners, etc.
Identify and mitigate patient\'s perceived barriers to therapy initiation and compliance through a customer care plan.
Document and maintain records of telephone interactions via electronic records.
Utilise motivational interviewing and cognitive behavioural therapy techniques to promote positive patient outcomes.
Coordinate and promote multichannel support materials based customized support plan discussed during phone interactions.
Participate in quality monitoring process where calls and electronic records are reviewed to ensure patient received the highest quality support possible.
Protect confidential patient health information in accordance with associated regulatory bodies.
Functional Skills and Knowledge
Excellent communication in English & Bahasa Malaysia, ability to communicate in Mandarin will be added advantage.
Enjoy developing an ongoing working relationship with patients throughout their treatment to ensure medication adherence.
Willing to travel as and when required
Education
Bachelor degree in any nursing or equivalent
Work Experience
Minimum one year of experience in supporting patients and caregivers in a clinical setting.
