45 hours per week with 1 Day Off



Flexibility in work arrangement is expected


MAIN PURPOSE / OBJECTIVES:




JOB SUMMARY





The Care Coordinator assists the Superintendent with the day-to-day running of the Home. She is expected to serve as a good role model and should be observant, attentive to the needs of the residents under our care, adopting a holistic care approach by looking into the physical,educational, social, emotional and spiritual needs of the residents.



The Care Coordinator is to ensure that she performs her job in compliance with The Salvation Army policies and procedures including the Territorial Child Protection Policy and all relevant government rules and regulations including those governing the Malaysia Care Centre Act.





MAIN DUTIES AND RESPONSIBILITIES




Assess and investigate new referrals that may involve home visits Propose admissions and recommend discharges to the Home Manager prior to such proposal or recommendation being presented at the monthly Residents' Review Board Engage in individual case work including counselling, family and group work Liaise closely with all staff and residents to identify needs and issues, present findings and recommend intervention proposals to Home Manager for preview and approval Conduct a 6-monthly periodic review of residents' progress and make recommendations at the monthly Residents' Review Board Prepare agenda and minutes of monthly Residents' Review Board as directed Provide guidance and advice to care and support personnel including volunteers as appropriate e.g behavior management of residents Maintain proper case file records for each resident and ensure that all documentation pertaining to residents are completed in accordance to the requirements of The Salvation Army Participate as a member of the Residents' Review Board of the Home and make recommendations on residents' care plans for Board deliberation and approval Education Coordinator support as additional responsibilities Supervise and co-ordinate study and recreational time Visit the children's schools with regards to children's educational needs and co-ordinate with other staff and superintendent Arrange tuition programme with volunteers or external agencies Provide written report on each child's progress and the tasks carried out for the child every 3months Network in general with other welfare agencies government bodies, with regards to the development of every child Encourage and motivate each child to live his/her life to its fullest Provide counselling where needed as educational guidance and support

Other Duties




Assist with supervision and provide mentorship to social work students attached to our social programmes as and when requested Provide constructive feedback and recommendations to management to enable best practices and service excellence to be upheld in our residential home and community service programmes Represent The Salvation Army in receiving donations, sponsored events, seminars and other events as and when required Attend conferences, workshops and training activities consistent with your position as and when required and approved by management Participate in staff meetings as and when required Undertake other duties and responsibilities consistent with the position as and when required








QUALIFICATIONS

Requirements:




Diploma/Degree in Social Work/Counselling/Psychology or any related field. Able to communicate in English, Malay & Mandarin/Cantonese. Driving Licence: D class and Other skills & abilities * Able to drive manual van is added advantage
